Noticeboard

From 1st February 2018 pharmacies and suppliers of medical appliances will no-longer be able to order repeat prescriptions on your behalf. PLEASE NOTE: Park & Calder Community Practice will not be able to accept prescription requests via telephone. If you have any specific needs, please speak to our reception staff, don't struggle, please ask us for help.

Prescription medication can be ordered from your GP by:

  • your GP’s online ordering service either by computer or a smartphone application,
  • by handing in the white, tear-off part of your repeat prescription to your surgery prescription box,
  • by letter.

New Patient Registration

You may register with us as an NHS patient if you are normally a resident in Calderdale.

You will be required to fill in a registration form and produce proof of address (i.e. utility bill, bank statement) and photographic evidence (i.e. current passport, driving licence).

As part of our registration process, you will be offered a health check with the nurse or health care assistant to discuss current health status, concerns and medication.

The three forms you need to complete are below - and you can download and complete them and bring to the surgery with two forms of ID as explained above. You can also collect these documents from our reception.
